Minimum Volume: 1.0 mL whole blood
This volume does not allow for repeat or additional testing.
Collection Instructions:

Invert tube 5-10 times.

If the type and screen if for a possible transfusion the patient must be given a Blood Bank wristband filled out with patient's name, medical record number or date of birth, draw date and time, location, and phlebotomist initials. Peel this label off of wristband and place on tube. Place wristband on patient.

Order questions will prompt Blood Bank to do the appropriate testing.

Rejection Criteria:

Hemolysis

A Blood Bank wristband is not issued to the patient for possible transfusion.

Specimen does not have a properly filled out Blood Bank identification wristband label on the tube if for a possible transfusion.

Additional Information: Additional sample may be required if antibody identification testing is indicated.  If additional testing is performed, additional charges/CPT code(s) may apply.
